We are seeking a qualified Physical Therapist to join our Home Health team! The Physical Therapist contracted or employed through the Organization is responsible for providing physical therapy services and for the adherence to all conditions outlined in the Personnel Services Agreements. New Grads are welcome to apply! Full Time or Per Diem.Responsibilities:Provide physical therapy services to patients according to a written physician's plan of care. This may include, but not limited to:Initial and on-going assessments to determine level of functioning, including OASIS assessments at appropriate time points. Assesses for muscle strength, mobility, gait, ROM- potential for rehab. Directing physical therapy treatment.Developing and revising the Plan of Care in consultation with physician and other health care team membersProviding therapeutic treatments and evaluation of equipment needs as applicableAdvising and consulting with the family and other agency personnel as applicablePreparing clinical and progress notes in accordance to agency policiesEvaluate outcomes of careSupervising Therapy Assistants and Home Health Aides as appropriatePlanning for dischargeInstructing patients and families/caregivers in the use and care of therapeutic appliancesReporting to physicians patient's reaction to treatment and changes in conditionInitiates physical therapy program and instructs other personnel and/orfamily/caregiver members in certain phases of physical therapyMay train patients in the use of prosthetic deviceQualifications:Possesses a degree for a baccalaureate or masters program in physical therapyLicensed to practice as a physical therapist in the State of CaliforniaTwo years of appropriate experience as a physical therapist.
