Our Company is in need of a polished and experienced Care Manager to handle prospective and current client care management in our San Rafael office.The Care Manager will be responsible for interphasing with prospective and existing clients in order to maintain their day to day service needs in a manner consistent with existing company policy. This individual must have the ability to exercise independent judgment within organizational guidelines while being aware of the impact of their decisions on the quality of clients’ service and the revenues of the company.The core functions of this position encompass the following: Provide comprehensive care management to assure that clients progress properly through the continuum of care provided by the organization.Coordinate with the hospital/SNF and family members for activities concerned with discharge planning and continued client care.Perform initial client assessments, client intake procedures and assist with implementation of care scheduling.Be available for referral source in-services and strategic business development meetingsAdhere to company goals, objectives, standards of performance, and policies and procedures.Ensure compliance with client care quality standards and regulatory issues.The position requires excellent time-management and multi-tasking skills, the ability to work well in a team atmosphere and a dedication to providing top notch customer service. The individual must be an excellent communicator with experience in making presentations, performing face to face client assessments, and communicating with medical professionals. Must be computer proficient.Associate, Bachelor’s, or Masters degree in a healthcare or social work related field preferred. Licensed RN or LPN preferred. Minimum of two (2) years of utilization review/case management experience or social work experience. Appropriate certification in case management is beneficial but not required.
